Our staff provides a wide range of services that help Leahi residents and Adult Day Health participants attain or maintain their highest practicable physical, mental, and psychosocial well-being. Our involvement starts from admission and continues throughout the stay of all residents/participants.
Upon admission a psychosocial assessment is completed to help identify strengths as well as areas of concern for each resident/participant. Our staff provides information and assistance with community resources, insurance (Medicaid QExA, Medicare D, etc.), Advance Directive/Surrogate Decision Maker, emotional support and discharge planning as needed.
We work closely with interdisciplinary team (IDT) members, residents and families to establish individualized care plans for all residents/participants. Quarterly conferences with the IDT provide residents/participants and family an opportunity to obtain updates and discuss questions or concerns. Our staff facilitates Resident Council meetings, which offer residents an opportunity to learn about updates occurring at Leahi and to present any questions or concerns they may have. We also facilitate quarterly Family Meetings, which provide an opportunity for family to meet staff; learn about specific topics; and obtain updates regarding staffing, projects, and other areas of concern. These meetings promote effective lines of communication between residents/families and staff. Participation in these meetings is always welcomed. In addition to the quality care that our staff provides, we also strongly encourage continued family involvement and visits, which in turn, helps to enhance the quality of life of our residents/participants.
