Community Services

Lifestyle – The types of patients we serve are those who are disabled and/or experiencing long-term care illnesses that are not able to care for themselves and have numerous health-care requirements.

Individuals who are unable to live independently in the community and need 24-hour nursing care.

  • The aged who are unable to care for themselves in their activities of daily living, including those with dementia, wheel chair bound and/or unable to control their bodily functions.
  • Individuals who need tuberculosis in-patient care.

Admission – Clients requiring long-term care are admitted either to a skilled nursing facility or intermediate care bed on referral from private physicians and various facilities within the state. Leahi Hospital’s staff, physicians and multidisciplinary team of health professionals provide treatment and general care.

In- House Services – We provide twenty-four hour skilled nursing, assistance with activities of daily living, health services, physical therapy, occupational therapy, and speech therapy. Our goal is to integrate custodial care with nursing, psychosocial, and rehabilitative services on a continual basis. A resident’s plan of care will differ based on his or her needs at the time of admission. Some residents will return home after a short rehabilitative stay following a hospitalization for an illness or accident. Others may require care for an extended period due to the chronic nature of their illness or disabilities.

Adult Day Health – Provides Adult Day Health, Monday through Saturday, to adults who are unable to care for themselves independently but who are able to participate in activities and live at home with their family. The service provides social and recreational/therapeutic activities, health observation and monitoring, supervision and assistance with personal care, medical treatments and medication administration, nutritious lunch and snacks, pharmacy medication delivery service, caregiver education and support, and bathing service.

Housing – Strict safety regulations are adhered to within this facility to accommodate the physical and mental disabilities of our residents.

Volunteer Services – Volunteers are welcomed to help with resident visitation, assisting with activities, transporting residents to and from activities and assisting with feeding residents their meals etc.
